Log in to NottinghamHub and select Request Support Plan tile to complete an online form.  At the end of the form you will be directed to attach supporting documents.  Short videos showing the steps are on this webpage.

 

The summary of the video guidance is:

  • Work through the form, selecting information from drop downs or toggling switches to complete the information. 

  • Under acknowledgments and consent, read the statements carefully and indicate your consent. If you select the option, no, I do not agree, in the first consent box, you can still submit your form, but disability support services will contact you before creating your support plan.

     

Valid file types include PDF, JPEG, PNG, Doc, Doc X, Excel, and email, EML or MSG types. File sizes can be a maximum of 25 megabytes, with a file name length a maximum of 60 characters.

 

To upload a document, click upload. If you are on a computer, you can drag and drop files into the screen, otherwise, click my device and locate the file on your device to upload. Once the file is uploaded, click done. To add more, click Add to add another row.

 

If you've attached any password protected documents, you must enter the document name and the password in the passwords field. Click submit.
